EAU CLAIRE, Wis. (WEAU)— The Community Health Assessment Partnership is holding community conversations in February.
The team is a collaboration of 9-partber coalition - health departments, health care facilities and nonprofit organizations in Chippewa and Eau Claire counties.
The goal is to work together to create change and improve the health of residents in Eau Claire and Chippewa counties.
A survey was conducted in November and December asking about your health concerns.
Sharing results will help identify, focus and target goals for the next several years.
