Fewer concussion forms would be required

MADISON, Wis. (AP) -- Parents would only have to sign one concussion information sheet a year for their child who participates in competitive sports under a proposal in the Legislature.

The bill up for a hearing Wednesday would change the state's year-old law passed to help better educate young athletes, parents, coaches and others about concussions.

That law required every sport participant under age 19 to have an information sheet about concussions signed by their parent or guardian. The form is required for every sport, so one child could have to get multiple copies of the same sheet signed in the same year.

The bill introduced by Sen. Paul Farrow would require a child to return only one form for the entire year, not a separate form for each sport.
